目的:探讨阿奇霉素序贯疗法联合布地奈德雾化吸入治疗儿童难治性支原体肺炎(RMPP)的疗效。方法:选取2023年2月—2025年2月我院收治的82例RMPP患儿,随机分为观察组与对照组,各41例。对照组采用阿奇霉素序贯疗法,观察组在对照组基础上加用布地奈德雾化吸入治疗。比较两组临床疗效、症状体征消失时间、炎症因子水平及不良反应发生情况。结果:观察组治疗总有效率为95.12%,高于对照组的80.49%(P<0.05);观察组发热、咳嗽、肺部啰音消失时间均短于对照组(P<0.05);治疗后,观察组C反应蛋白(CRP)、降钙素原(PCT)、乳酸脱氢酶(LDH)水平低于对照组(P<0.05);两组不良反应发生率无统计学差异(P>0.05)。结论:阿奇霉素序贯疗法联合布地奈德雾化吸入治疗提升RMPP患儿临床疗效,缩短症状缓解时间,降低炎症反应,且安全性良好。
Abstract:Objective: To explore the efficacy of sequential azithromycin therapy combined with budesonide inhalation in the treatment of refractory mycoplasma pneumonia(RMPP) in children. Methods: A total of 82 children with RMPP admitted to our hospital from February 2023 to February 2025 were randomly divided into an observation group and a control group,with 41 children in each group. The control group was treated with sequential azithromycin therapy,while the observation group was treated with budesonide inhalation in addition to the treatment of the control group. The clinical efficacy,disappearance time of symptoms and signs,levels of inflammatory factors,and occurrence of adverse reactions were compared between the two groups. Results: The total effective rate of the observation group was 95.12%,which was higher than 80.49% of the control group(P<0.05); the disappearance time of fever,cough,and pulmonary rales in the observation group was shorter than that in the control group(P<0.05); after treatment,the levels of C-reactive protein(CRP),procalcitonin(PCT),and lactate dehydrogenase(LDH) in the observation group were lower than those in the control group(P<0.05); there was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05). Conclusion: Sequential azithromycin therapy combined with budesonide inhalation can improve the clinical efficacy of children with RMPP,shorten the time of symptom relief,reduce the inflammatory response,and has good safety.
