目的：探究基于辨证的穴位注射疗法与标准化中成药治疗黄褐斑的临床疗效及安全性，为黄褐斑的临床治疗提供更优的方案参考。方法：选取2022年12月—2024年12月郑州韩辰医疗美容医院皮肤科收治的102例黄褐斑患者作为研究对象，按照随机数字表法分为对照组和观察组，每组51例。对照组采用标准化中成药治疗，观察组采用基于辨证的穴位注射疗法。比较两组临床疗效、皮损面积及颜色评分、血清氧化应激指标[丙二醛（MDA）、超氧化物歧化酶（SOD）]、内分泌指标[雌二醇（E2）、孕酮（P）]水平及不良反应发生情况。结果：观察组治疗总有效率高于对照组（P<0.05）；观察组治疗后MASI评分均低于对照组（P<0.05）；观察组血清MDA水平低于对照组，SOD水平高于对照组（P<0.05）；观察组治疗后血清E2、P水平低于对照组（P<0.05）；两组不良反应发生率比较，差异无统计学意义（P>0.05）。结论：基于辨证的穴位注射疗法治疗黄褐斑的临床疗效优于标准化中成药，能有效改善患者的皮损情况，调节血清氧化应激及内分泌指标，且安全性较好。

Objective: To explore the clinical efficacy and safety of acupoint injection therapy based on syndrome differentiation and standardized Chinese patent medicine in the treatment of melasma, and to provide a better scheme reference for the clinical treatment of melasma. Methods: A total of 102 patients with melasma admitted to the Department of Dermatology, Zhengzhou Hanchenhualing Medical Beauty Hospital from December 2022 to December 2024 were selected as the research objects. According to the random number table method, they were divided into control group and observation group, with 51 patients in each group. The control group was treated with standardized Chinese patent medicine, and the observation group was treated with acupoint injection therapy based on syndrome differentiation. The clinical efficacy, skin lesion area and color score, serum oxidative stress indexes [malondialdehyde(MDA), superoxide dismutase(SOD)], endocrine indexes [estradiol(E2), progesterone(P)] and adverse reactions were compared between the two groups. Results: The total effective rate of the observation group was higher than that of the control group(P<0.05). After treatment, the MASI score of the observation group was lower than that of the control group(P<0.05). The serum MDA level in the observation group was lower than that in the control group, and the SOD level was higher than that in the control group(P<0.05). After treatment, the levels of serum E2 and P in the observation group were lower than those in the control group(P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05). Conclusion: The clinical efficacy of acupoint injection therapy based on syndrome differentiation in the treatment of melasma is better than that of standardized Chinese patent medicine. It can effectively improve the skin lesions of patients, regulate serum oxidative stress and endocrine indicators, and has good safety.
